About this Facility:
Celebrating 25 years of providing quality healthcare to Davidson, Wilson and surrounding counties, TriStar Summit is a 218-bed comprehensive facility offering a full array of acute care services, including cardiology, emergency, oncology, orthopaedics/spine, rehabilitation and women’s services. As an Accredited Chest Pain Center with PCI and a Primary Stroke Center, TriStar Summit is a leading provider of emergency heart and stroke care. There are more than 300 board-certified physicians representing over 36 specialties on staff, many of whom have offices located on TriStar Summit’s campus.
About this opportunity:
The Case Manager will address the discharge planning needs and the psychosocial needs of the patient population served by the facility, and those patients referred through an open referral policy as well as those referred through high risk referral criteria. The Case Manager will render assessment, community service coordination and assist with discharge planning to facilitate continuity of care and the transition of patient with personal/environmental difficulties that may interfere with obtaining maximum benefit from medical care. The Case Manager is responsible for maintaining standards of professional and ethical practice in the provision of case management services and facilitates interdisciplinary team approach to meeting the broad spectrum of the patient’s healthcare needs.
Responsible for the appropriate allocation of hospital’s resources through effective and efficient case management consistent with JCAHO standards, governmental regulations, and third party payer requirements. Monitors and reports identified trends in practice patterns that are not consistent with established criteria for admissions/discharges or departures/transfers. Responsible for daily monitoring of appropriateness of transfers, admission status, and length of stay, and the appropriateness and timeliness of post-discharge/departure symptom management through access of community resources. Facilitates interdisciplinary team approach to meeting the broad spectrum of the patient’s healthcare needs.
