JOB SUMMARY:
Assist with the delivery of patient care and maintenance of unit environmental standards under the supervision of a licensed nurse.

Your Role:

As a Care Partner/Home Health Aide with Vanderbilt Home Care Services, you will travel to the patient home’s to provide the precribed care. Nursing Assistant Certification is required. Position pays a competitive hourly rate and mileage allowance.

Position Shift:

  • 1 full time (30 to 40 hour week)
  • 2 part time (24 to 30 hour week)

The Home Health Quality team is a small task oriented team which consists of a quality educator, a quality data analyst, and quality reviewer. They work closely together with clinical managers to provide meaningful data on home health patient population and clinician best practices.

KEY RESPONSIBILITIES:

  • Assist with the delivery of patient care and maintenance of unit environmental standards under the supervision of a licensed nurse.
  • Assists with day to day delivery of patient care.
  • Facilitates efficient and safe patient care and flow by following work area standards.
  • Serves as a liaison for communication and education of patient and family needs.
  • Supports unit operations and patient flow by maintaining supplies, equipment and unit environmental standards.

Position Requirements:

  • High School Diploma or GED
  • 1 year relevant experience

Licensure, Certification, and/or Registration (LCR):

  • Nursing Assistant Certification
  • Active drivers license
